Terms and Conditions of Use

UMCNO is pleased to offer our patients online access to portions of their medical record through the MyChart web portal (“Site”). Use of this service is optional. Patient access includes records created on the date MyChart service is activated, as well as records that are created after the date the activation process is completed.

At this time, UMCNO provides the following services through MyChart:

  • Request or cancel an appointment
  • View and/or download selected portions of your medical record
  • Receive selected lab and/or radiology results
  • Communicate with your health care provider via secure messaging regarding health concerns
  • Access (through links) to other sites which can provide additional health information

Your Responsibilities:

By accessing or using UMCNO’s MyChart you agree to be bound by all the terms and conditions of this Agreement as posted. UMCNO may modify this Agreement at any time, and you, as a user of the LSU MyChart agree that such modifications are effective immediately upon posting of the modified version. If you do not agree to be bound by these terms and conditions, please promptly exit this Site.

To enroll in UMCNO’s MyChart, you will use your access code which appears at the bottom of your visit’s After Visit Summary. The access code you receive will give you the opportunity to create a personal login name (called “MyChart Username”) and a password. Once this access code has been used or after 60 days of assignment, it will no longer be valid.

Your login name/ID and password provide two layers of protection and authentication: evidence of identity and an encrypted secured database that is isolated from the Internet. Your password is not known by anyone at UMCNO. As a MyChart user your role in maintaining the security of your medical information is: 1) Changing your password on a regular basis and 2) Keeping your login ID and password confidential.

It is extremely important that you keep your login name and password completely confidential. Anyone with access to your login name and password will be able to access your confidential medical information. That person will be able to read your messages and send new messages as if they were you. It is your responsibility to prevent the disclosure of your Login name/ID and password, and to change your password if you feel that your security has been compromised. You may change your password at any time.

Be aware that sensitive information that you may not want others to see may be contained in MyChart. Information such as substance abuse, HIV status, cancer diagnoses, etc. may be posted in MyChart. Please keep this in mind when you give your significant others access to your MyChart account. While this may not be of concern to you now, should you ever be tested for or diagnosed with a sensitive diagnosis, persons who have access to your MyChart account will be able to see this information. If you do not want anyone to see this information, you may want to restrict access to MyChart from others.

Additionally, if UMCNO discovers that you have inappropriately shared your password or that you have abused your access privileges, UMCNO may terminate your participation in MyChart. UMCNO takes no responsibility for and disclaims any and all liability for damages arising from a breach of confidential medical information resulting from sharing or mishandling your password.

Electronic Messaging/Communications - Medical Questions:

  • UMCNO’s MyChart should NEVER be used for urgent matters. The anticipated turnaround response to secure electronic messages is 3-5 business days. For all urgent medical matters, please contact your physician’s office by phone, go to your local emergency room or dial 911.

    Please know that if you send us a health concern message, it may be shared with UMCNO staff that assists the health care provider in order to facilitate a timely response to your message. A patient’s confidential medical information on MyChart will be accessible only by appropriate staff.

    Furthermore, with respect to any electronic communications sent by the patient, health care providers are only able to respond to such communications based on the information provided by the patient.If there is insufficient information provided, health care providers will be unable to provide accurate and reliable services.
  • All communications between you and health care providers using MyChart occur over a secure connection. You will receive Internet email messages notifying you that new information is available in your MyChart account. These email messages will not contain any confidential medical information.

    However, this means that any person with access to the patient’s email will be able to see this notification. Patients should take this into account when providing an email address.
  • Since messages sent to a health care provider may become part of your permanent medical record and will be perceived as your health information, it is important that no one else’s information be reflected in any messages. With authorization, information from your medical record may be released to insurance companies when applying for insurance. UMCNO takes no responsibility for erroneous medical information that you provide via MyChart that may impact your ability to be covered by insurance.

Proxy Access
A proxy is a person authorized to act for another. UMCNO offers a variety of proxy accesses. If you are interested in this feature, please inquire in the clinic where the patient receives care. Available Proxy Access may include:

  • Adult (Parent/Legal Guardian) access for children (newborn to 12th birthday)
  • Adult as Guardian/Other Court Appointed Title Adult with Health Care Power of
  • Attorney (HCPOA) for another adult
  • Adult (Notarized Letter) for another Adult/Spouse

Your privacy is of the utmost importance to us at UMCNO. Health care providers will use your confidential medical information in order to provide your health care services. Health care providers will, at all times, maintain your confidential medical information in strict confidence and will not disclose any information regarding you and your health information to any unaffiliated third party unless you have authorized that person to receive your information or the disclosure of the information is permitted by law. All messages sent and received within MyChart that contain medical information are subject to all state and federal laws governing the security and confidentiality of medical records.

Please review UMCNO’s Notice of Privacy Practices (NPP) for a thorough description of how we gather, use and protect your confidential medical information. Copies of the NPP are available at each UMCNO facility.

Security and Confidentiality
At UMCNO, we limit employees’ access and the ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption and audit logs are further used to safeguard your information. UMCNO has a process to identify the records viewed/accessed and note the time and date of view/access each time a login of MyChart occurs. UMCNO has taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use.

For other than general information viewing, MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal. Our SSL web server uses authentication and offers the highest level of encryption technology commercially available.

You can tell when you are secure by looking at the location (URL) filed. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. The certificate verifies the connection between UMCNO’s server’s public key and the server’s identification.

Accuracy and Integrity of Information
Although UMCNO attempts to ensure the integrity of the Site, it makes no guarantees whatsoever as to the correctness or accuracy of the Site. It is possible that the Site could include typographical errors, inaccuracies or other errors.In the event that you identify an inaccuracy, please inform UMCNO’s Health Information Management so that it can be corrected or amended. Information contained on the Site may be changed or updated without notice.

Information Does Not Constitute Medical Advice
UMCNO assumes NO responsibility for how you use the information you obtain from this Site. You should consult with a physician or other health care provider regarding your own condition and how this Site content may or may not apply to you.

Restrictions on Use and Ownership
All pages within this Site and any material made available for download are the property of UMCNO and/or its subsidiaries and affiliates. The content on this Site is provided for UMCNO and its patients only. The words “UMCNO” and its associated logo are the trademarks and service marks of LCMC Health, part of the Board of Supervisors of Louisiana State University and Agricultural and Mechanical College.

Termination of Access
You may end your usage of UMCNO’s MyChart at any time by contacting the Health Information Management Department at the UMCNO facility where you signed up.

UMCNO may restrict or terminate access to MyChart at any time. If access is restricted for planned maintenance of the MyChart system, every effort will be made to communicate downtime to all users in advance. However, there may be reasons to restrict access to MyChart without advance notice. If we determine, in our sole discretion, that your use of MyChart violates these Terms and Conditions, UMCNO reserves the right to permanently terminate access to MyChart. You will be notified if UMCNO terminates your access.

Links to Other Sites
UMCNO’s MyChart may offer links to related medical websites not managed by UMCNO. These website link(s) are for informational purposes only. UMCNO does not endorse, and has not verified the accuracy of the information in/on these websites and you should not rely on any of the information found on the websites for purposes of treatment or diagnosis.

This agreement shall be governed by and construed in accordance with the laws of the State of Louisiana.

The terms hereby used in this agreement are used in their generally prevailing meaning unless defined hereafter.

  • LCMC Health: includes but is not limited to all LSU System hospitals, in-patient and out-patient clinics, mobile units and triage centers.
  • Healthcare Providers: includes but is not limited to physicians, residents, fellows, registered nurses, licensed practical nurses, physician assistants, nurse practitioners, nurse assistants, pharmacists, occupational therapists, physical therapists, speech therapists, audiologists, rehabilitation counselors, social workers, psychologists, dentists, dental hygienists, lab and radiology technologists, and others employed by or affiliated with UMCNO.
  • Health Care Power of Attorney (HCPOA): is a legal document that allows an individual to designate another person to make medical decisions for him or her when he or she cannotmake decisions for himself or herself.
  • Guardian: is a person who has the legal authority and duty to care for another person especially due to the other person’s age, incapacity, or disability. A guardianship is an involuntary trust relationship in which one party, called a guardian, acts for an individual called the ward.
  • Notarize: the act of officially certifying a legal document by a notary public. The purpose of having a legal document notarized is to ensure the authenticity of the signatures that appear on the document.
